Steps to Health community health specialists help people with diabetes or asthma to stay healthy. We provide free home visits to support people in self-managing their disease, working with providers and reducing asthma causing triggers in their home. We can work with anyone speaking Vietnamese, English or Spanish and look forward to working with you soon.
