Agency-based program pointers
A note to our partners in the agency-based program
Our thanks to all the people at the
agencies we’re working with for turning in your clients’
applications. Please tell your clients that it is important that
they answer every question on the application. If the
application asks whether the client has a bank account and the
client doesn’t have one, for example, don't leave the question
blank; instead, write in “zero” or “N/A” so that we know the
question wasn’t just missed. If a question requires a “yes” or “no”
answer, please don't allow your client to write in “N/A.”
A few other pointers:

Every client must sign and date
the citizenship form. (Writing “N/A” on the form isn’t
acceptable.)

Always include a legible copy of
state ID for every adult in the family (or a copy of their
Permanent Resident card), and always include a copy of every
family member’s Social Security card.

If any adult in the family has no
income at all, that person must complete the Zero Income
Affidavit and must also provide verification that he or she
receives no Social Security benefits.

If your client worked in 2006 but
is not currently working, we require a letter from his or her
previous employer stating that your client no longer works for
them and indicating their last day of work. This letter must be
on the employer's letterhead and bear the name and phone number
of the person writing the letter.
